The image is a tweet, likely from a social media user named Wolvi, with a screenshot of a news report. The report shows Turkish President Erdoğan's wife, Emine Erdoğan, giving a seminar to medical professionals. The tweet's text is a sarcastic comment.
The joke plays on the perceived lack of qualifications of Emine Erdoğan to give a seminar to medical professionals, in this case. The tweet says "İlkokul mezunu Emine Erdoğan tıp insanlarına seminer veriyor." (Emine Erdoğan, who only finished elementary school, is giving a seminar to medical professionals.) This implies that she isn't qualified to give a seminar on such a complex topic as medicine.
It's a humorous jab, using the fact that she only finished elementary school in her youth, in comparison to the highly specialized nature of the subject of medicine.
